We have created two different Instrument Maintenance Forms: the choice is yours!
This forms should be used before releasing any instruments to students.
The first allows you to enter data/information by typing into fill-able fields on a Word Doc. Choose the form entitled:
Form Maintenance.doc (Directions are on the bottom of the form)
Please fill in all of the fields except pd by/date before sending home with the students.
If you prefer a paper form, with hand-written information, use the form entitled,
"Instrument Maintenance Form_Long Form.doc (Directions for submission are on the bottom of the form)
Updated 9_3_2017 to include Spanish and Vietnamese forms.
