PERKINS DISPOSITION FORM
Please fill out the Perkins Equipment Disposition form (attached). Complete the form as completely as possible, including all relevant details.
Then, create a New Ticket and submit the ticket along with the completed form.
Include as many details as possible. If unsure if the item is Perkins, go ahead and fill in the form and send it. You can use one form for a quantity of items (such as 30 computers that were removed and recycled over the summer).
